What ongoing services do I need to have?

For many people with diabetes, basic treatment may involve checking blood sugar and/or medication. You and your providers should create a personal care plan, but here are a few services you should have.

Blood Sugar Monitoring

A1C is a blood test that shows average blood sugar (glucose) levels over the last 3 months. The test is used to see how well a person with diabetes controls their blood sugar. The test can help guide diabetes treatment over time. The ADA says that people with diabetes should keep an A1C below 7%, but personal goals may vary. Learn more

Eye Care

Diabetic retinopathy is the most common eye disease in people with diabetes, and is the leading cause of blindness in American adults. Have a dilated eye exam once a year. An eye exam doesn’t prevent retinopathy. But it can help diagnose eye problems that can then be treated. An eye exam can also alert you and your health care provider if the diabetes needs to be better controlled. Learn more

Kidney Health

Diabetic kidney disease is kidney disease that is due to diabetes. It is also called diabetic nephropathy. Nephropathy means your kidneys aren’t working well. Have your urine tested every year to check for a protein called albumin. Even a small amount of albumin in your urine is a sign of early kidney damage. Learn more

Advantage MD Resources

Care Management

Advantage MD members with diabetes can work with a care manager for extra support. Your care manager will help you understand your condition and work with you to adopt better eating and exercise habits. They can also work with your providers to bridge your care. Learn more

Health Education

Members have access to no-cost classes focused on managing diabetes. Our Health Education program also has classes on improving your eating habits, being more active, reducing stress, and many other topics. Learn strategies to better control your condition and make healthy changes that last. Learn more
