If you choose to provide us with personal information by sending an email, or by filling out a form with your personal information and submitting it through our Web site, we use that information to respond to your message and to help us provide you with information or material that you request. Sometimes, managing all of your health care needs may be too much for you. When that happens, ask for a care manager. These nurses and social workers can help you better understand and manage your health conditions. They can help coordinate your care with your providers. They can make it easier for you to achieve your best health.
Partner with us to stay healthy and avoid complications. You’ll get health information you can trust and reminders about services you need. You may also be able to work with a care manager, who can work with your providers, help you understand a new diagnosis, and use your plan.
Get extra support when you have a change to your health. The change could be temporary (an injury or inpatient stay) or long-term (new treatment). We help you adjust to these changes, support you with your specific needs, and guide you back to your best health.
Learn to manage your chronic conditions. We work with you and your providers to help you understand and take better care of your complex health conditions. We help coordinate your care and work one-on-one to make it easier for you to focus on your health.
