Effective March 21, 2022, there will be a new address and fax number for submitting appeals to Johns Hopkins Advantage MD. This important change of contact information (below) only applies to the process of filing appeals.
Why is this important to you? As a member of Johns Hopkins Advantage MD, you have a right to request a coverage decision. You also have a right to file an appeal if you don’t like the coverage decision. For example, we make a coverage decision, but you disagree with it. You can then “appeal” the decision, which is just a formal way of asking us to look at it again and change the decision.
Any appeals need to be made within 60 days from the date of the coverage decision, and if you’re not happy with the answer to your appeal, you can appeal that decision. Of course, if you need assistance, you may name someone to act for you as a representative to submit your appeal — a relative, friend, lawyer, advocate, doctor or someone else are all acceptable. Also, all appeals requests must include a completed and signed Waiver of Liability Statement form. Learn more about when, how and where to submit appeals here.
