Medication Guidance: A Real-Life Member Story
“Sharon” had missed one of her medicine refills, so an AdhereHealth Care Navigator called her to find out how they could help. Sharon explained she was having trouble remembering to take her pills and couldn’t always get to the pharmacy to pick them up. She also said she often ran short on money to buy enough food. Her daughter was able to help her sometimes, though not always.
Working with AdhereHealth, Sharon was able to sign up for mail order pharmacy, so her medicine would be delivered right to her door. They talked with her about keeping up with her medicine schedule by using a weekly pill planner. AdhereHealth’s Care Navigator also connected her with a few free community resources for food, such as the local Meals on Wheels, and transportation from the local Senior Services Center.
