In-Home Health Assessments
It’s OK — stay home. With this perk from Advantage MD, a licensed clinician will come to you. During this no-cost visit, members will spend up to one hour with a clinician to have their medication and medical history reviewed, get their vital signs checked and ask questions. This is an extra visit beyond your wellness visit. Members eligible for this visit may also receive a $25 gift card. Virtual visits are also available. If eligible, Advantage MD or a representative from Signify Health will contact you.
