That means making sure you get the care you need, when you need it, including preventive care and in-home care options. But we wouldn’t be able to do all the things we do to support your health journey without a little help from our friends — our trusted vendors. Please know that we work with your primary care providers when we offer you these services.
For the best service, these vendors may contact you directly through mail, phone calls, emails or text messages.
Our vendors help us to save you money on your health care costs and deliver the highest quality health care.
Signify
Signify provides no-cost in-home health assessments from the comfort of home. Virtual visits are also available. This is an extra visit beyond your wellness visit. Eligible members may also receive a $25 gift card.
If eligible, Advantage MD or a representative from Signify Health will contact you to:
- Arrange up to a one-hour visit with a licensed clinician
- Review your medication and medical history
- Check your vital signs and answer any questions
AdhereHealth
AdhereHealth reviews eligible members’ medications and coordinates with their health team to optimize their medication regimen.
This is a no-cost service. Certain eligible members may receive a phone call from an AdhereHealth clinician who can:
- Find ways to save money on prescription costs
- Identify potential medication side effects
- Verify proper medication use
- Explore free support services
