
If your clients did not choose a PCP during enrollment—or if they want to change their PCP—they must call Customer Service at 1-877-293-4998 before January 1, 2026.
This is especially important for HMO plans. If a PCP isn’t selected, one will be automatically assigned, which may cause avoidable issues with referrals or care coordination later.
Please remind your clients now so their PCP choice is correct and active heading into the new plan year.
